Patients with low body weight (less than equal to 60 kg.
moderate to severe CHF (NYHA class III-IV); lack of CYP2D6 activity; congenital or family history of long QT syndrome, known acquired QT interval prolongation & those treated with drugs that affect the QTc interval.
Not for use in patients treated with CYP3A4 inducers (e.g., rifampicin, phenytoin, phenobarb, carbamazepine, St. John's wort).
Patients with rare hereditary problems include galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. The dye E102 can cause allergic reactions. Check kidney function regularly during treatment.
Concomitant use of moderate CYP3A4 & P-gp inhibitors. Mild hepatic impairment & mild to moderate renal impairment (CrCl 30-80 mL / min).
May cause dizziness, blurred vision, diplopia, confusion, abnormal coordination, hallucinations which may affect ability to drive & use machines.
Pregnancy & lactation. Children <18 years. Elderly.
