- When contraception is not used
- When a contraceptive method fails, including:
- The condom is torn, slipped or used incorrectly

- Failure to have interrupted intercourse
- Miscalculation in the calendar method
- IUD removal
- Forgetting to take contraceptive pills for three or more days in the cycle
- In rape cases
